Ban Report/Appeal General Info & Format

Rules - Alright, I only have three rules for these appeals.

1. The report of the player/admin must be tied to them breaking the rules in some way or another.
2. No crashing threads and no begging. You will be forum banned for either.
3. Must have screenshot proof of some sort.
